Municipal Sport Facility Request Form
The information you provide on this form will allow the Facility Booking Team to check availability and expedite the booking process. Please note that by submitting this request form, it does not confirm your booking. The rental is not finalized until a Certificate of Insurance is received and approved, the contract is signed, and payment is made.
Payment is due in full at the time of booking. For seasonal bookings, payment is due in advance on the 1st of each month.

| Note: Effective May 6, 2024 our regular office hours will be Monday to Friday 7:30 a.m. to 3:30 p.m. Rental requests received outside of regular office hours, on weekends or holidays will be processed in sequence of requests received upon the office reopening. Please note a completed Request Form is not considered a booking. |
